Many cases of male infertility are genetically determined, and thus not currently possible to correct. Some of our male patients with low sperm count or motility fall into this category of male infertility. Therefore, our infertility team works with the couple in to increase their general reproductive efficiency. Intrauterine insemination or in vitro fertilization are the most probable responses to the condition of male infertility, with a few other options depending on the individual patient. In rare cases, medications can help to improve hormonal deficiencies.
Varicocele ligation for male infertility
A variocele is an abnormal twisting and dilation of veins within the spermatic cord. A variocele can be treated surgically, however studies are still in dispute of the benefits of such intervention. Pregnancy success rates for some patients with male infertility have improved after variocele ligation, while in others lower pregnancy rates have occurred. Clomiphene citrate (Clomid, Serophene) for male infertility
In attempt to improve the semen quality of patients that are diagnosed with male infertility (semen abnormalities), some patients can be treated with clomiphene citrate prescriptions, normally in the form of pills.
Clomid can improve the semen motility or count, yet medical literature continues to prove that there is not a relevant increase in pregnancy rates.
A very interesting study was published by the WHO on 190 couples that were given a clomiphene regiment or a placebo (sugar pills) in men diagnosed with male infertility or abnormal spermatozoa (International Journal of Andrology, 1992). Each male received the medication or placebo for 6 months. The study demonstrated that in couples who took the medication, pregnancy rates increased by 8.1%. Intrauterine insemination and in vitro fertilization with male infertility
In patients with male infertility, in particular with mild to moderate problems with the quality of their spermatozoa, ovarian stimulation complimented with intrauterine insemination (IUI) can help with pregnancy rates. When problems increase with sperm abnormalities, men can sometimes be treated with intrauterine insemination (IUI), however in vitro fertilization (IVF) is usually the best means of conception. Exaggerated problems of sperm quality, count or motility may demand intracytoplasmic injection (ICSI) in conjunction with in vitro fertilization (IVF), or for some, donor sperm - depending on the preferences of the couple.
